Below list positions available:

  • Account Executive(s) - The home care sales & marketing professional establishes a professional relationship with Physicians, discharge planners, social workers and other health care decision makers by providing on-going education to them regarding home health services and care. The home care sales & marketing professional meets professional development goals through the management of an assigned territory, professional relationships and knowledge of home health.
  • RN Case Manager(s) with OASIS C experience - to provide, coordinate, and direct home care services in the field. Per visit rates or full time.
  • LVN Field Nurse(s) - coordinate care with other interdisciplinary teams, patient/family, and the referring agency . This position is under the direct supervision of the RN.
  • Home Health Aide/Provider - Provide routine, personal healthcare, such as bathing, dressing, or grooming, to elderly, convalescent, or disabled persons in the home of patients or in a residential care facility.
